I explore some of the ways that assumptions about the nature of substance shape metaphysical debates about the structure of Reality. Assumptions about the priority of substance play a role in an argument for monism, are embedded in certain pluralist metaphysical treatments of laws of nature, and are central to discussions of substantivalism and relationalism. I will then argue that we should reject such assumptions and collapse the categorical distinction between substance and property.

The debate over the temporal arrow is a debate over what fundamental ontology is needed for the temporal asymmetry of the universe, which determines the fact that time seems to be oriented or directed from earlier to later. This temporal asymmetry underlies (or, as some might argue, is the same as) the asymmetrical fact that the past is fixed while the future is open, as well as the global asymmetries of counterfactual, causal and agential direction. I explore the metaphysics of (...) the temporal direction within the context of empirical work in psychology and cognitive science. I argue that, in our debate over the metaphysics of the temporal arrow, we need to explore some of the deep connections between our cognitive response to the world and our metaphysical theory of the temporal direction. I claim that Mill has a theory of poetry which he uses to reconcile nineteenth century associationist psychology, the tendency of the intellect to dissolve associations, and the need for educated members of society to desire utilitarian ends. The heart of the argument is that Mill thinks reading poetry encourages us to feel the feelings of others, and thus to develop pleasurable associations with the pleasurable feelings of others and painful associations with the painful feelings of others. Once the associations (...) are developed, they are supported and maintained by our natural capacity for sympathy and by external elements in society, and provide motivation for the pursuit of utilitarian ends. Further, the additional support causes the associations to be strengthened to the extent that they come to be seen as ‘natural and necessary’, and as such are immune from the dissolving force of the intellect. Aim: Patients with advanced cancer need information about end-of-life treatment options in order to make informed decisions. Clinicians vary in the frequency with which they initiate these discussions.Patients and methods: As part of a long-term longitudinal study, patients with an expected 2-year survival of less than 50% who had advanced gastrointestinal or lung cancer or amyotrophic lateral sclerosis were interviewed. Each patient’s medical record was reviewed at enrollment and at 3 months for evidence of the discussion of patient wishes concerning (...) ventilator support, artificial nutrition and hydration , resuscitation and hospice care. A Kaplan–Meier analysis was also performed and 2-year survival calculated.Results: 60 cancer and 32 ALS patients were enrolled. ALS patients were more likely than cancer patients to have evidence of discussion about their wishes for ventilator support , ANH , DNR and hospice care . At 6 months, 91% of ALS patients were alive compared with 62% of cancer patients; at 2 years, 63% of ALS patients were alive compared with 23% of cancer patients .Conclusions: Cancer patients were less likely than ALS patients to have had documented advanced care planning discussions despite worse survival. This may reflect perceptions that ALS has a more predictable course, that advanced cancer has a greater number of treatment options, or differing views about hope. Nevertheless, cancer patients may be less adequately prepared for end-of-life decision-making. (shrink)

There is a very sharp upsurge of interest in the West in the history of Russian conservative thought, particularly in its most outstanding figure, K. Leont'ev. Judge for yourself. In 1948 a monograph on him appeared in West Germany ; one appeared in the USA in 1952 ; and in Italy in 1957 . In 1966 he was inscribed in the "family of the very greatest Russian intellects," to which his large book, Scrittori Russi is devoted. Added to this is (...) N. Berdiaev's book K. N. Leont'ev, which has been translated into English and French, V. Zenkovsky's entire chapters on Leont'ev in his History of Russian Philosophy [Istoriia russkoi filosofii], 1948, and in his book Russian Thinkers and Europe [Russkie mysliteli i Evropa], 1955; and the chapters by the American historian E. Thaden, in his Conservative Nationalism in 19th Century Russia, Seattle, 1964. It thus becomes clear that Leont'ev has been forgotten in a strangely one-sided fashion. As we see, in the West he is not only not forgotten, but his ideas are, ever more obviously, becoming a powerful weapon in a very current ideological struggle. Superficial books written about him find a market. His thoughts are made the basis of very contemporary and dangerous notions. It turns out then that he has been forgotten only in our country. AFTER ARGUING THAT LAUDAN’S ACCOUNT OF THE ROLE OF CONCEPTUAL CONSIDERATIONS IN THEORY APPRAISAL IS INADEQUATE AND UNSATISFYING IN A NUMBER OF RESPECTS, I SUGGEST SOME OF THE WAYS IN WHICH WE MIGHT MOVE TO DEVELOP AN ALTERNATIVE ACCOUNT. THIS ALTERNATIVE PRESUPPOSES A PROBLEM-SOLVING METHODOLOGY AND, UNLIKE THE LAUDANIAN APPROACH, AWARDS A CRUCIAL ROLE TO EMPIRICAL RESEARCH IN THE RESOLUTION OF THE CONCEPTUAL PROBLEMS TROUBLING A THEORY. THREE WAYS IN WHICH A THEORY MAY ENHANCE THE CONCEPTUAL RESOURCES WHICH IT SUPPLIES (...) FOR EMPIRICAL PROBLEM SOLVING ARE CONSIDERED: THE FINE-TUNING OF THEORETICAL CONCEPTS; THE APPROPRIATION OF THE CONCEPTUAL RESOURCES OF THEORIES IN OTHER DOMAINS; AND THE ACHIEVEMENT OF GREATER CONSILIENCE. REFERENCE IS MADE TO SEVERAL HISTORICAL CASES IN WHICH SUCH ENHANCEMENT ACTUALLY OCCURRED. (shrink)
